When applying for Permanent Residence in Japan, you must submit not only the application form and guarantor-related documents, but also an Acknowledgment Form.
This form confirms that, from the time you submit your permanent residence application until you receive the result, you must promptly report important changes to the Immigration Services Bureau, such as changes in employment, family status, tax or social insurance payments, or criminal matters.
As of March 23, 2026, the Immigration Services Agency of Japan still lists the Acknowledgment Form as a required document for permanent residence applications. If you are preparing a PR application, make sure it is included in your application package.
What is the Acknowledgment Form?
The Acknowledgment Form is a document in which the applicant confirms that they understand their obligation to report material changes during the screening period for permanent residence.
PR screening often takes a long time. Because your circumstances may change while the application is under review, immigration authorities examine not only your situation at the time of filing, but also your status and compliance during the review period.
What kinds of changes should be reported?
Typical examples include the following:
- Resignation, job change, transfer, leave of absence, or other employment-related changes
- Marriage, divorce, separation, changes in household members, or other family-related changes
- Changes in the payment status of taxes, pension contributions, or public health insurance premiums
- Traffic offenses, criminal penalties, or other legal violations
- Major changes in address or living circumstances that may affect the screening
It is risky to assume that a change is “too minor” to report. If you are unsure, it is safer to consult a qualified professional or check with the immigration office handling your application.
Why this form matters
Permanent Residence is a highly important immigration status in Japan because it removes the need for future period-of-stay renewals. For that reason, the screening process carefully reviews tax compliance, pension and insurance payments, conduct, and stability of livelihood.
The Acknowledgment Form is not just a formality. It is a written confirmation that you understand your obligation to report relevant changes after filing. Failing to do so may negatively affect your application.
Practical points before and after filing
- Check your tax, pension, and health insurance payment records before applying
- If you plan to resign or change jobs, carefully consider the timing of your PR application
- If your family situation may change soon, organize the facts in advance
- If something changes during the review, do not leave it unattended
- Required documents differ depending on your status category, so review the full document list carefully
Multilingual versions are available
The Acknowledgment Form is also available in multiple languages, including Easy Japanese and English, which helps applicants better understand the contents before submission.
However, PR applications require much more than this single form. Depending on your category, you may also need tax certificates, proof of pension payments, health insurance records, guarantor documents, and other supporting materials.
